Some patients may switch between Mounjaro® (tirzepatide) and Wegovy® (semaglutide) for reasons such as availability, side effects, treatment response, cost, or clinical suitability.
There is no exact dose-for-dose conversion between Mounjaro and Wegovy because they are different medicines. The table below is a general estimated guide only. The most appropriate starting dose after switching will depend on:
Both medicines are usually injected once weekly and are increased gradually to help improve tolerability. Mounjaro is normally increased in 2.5 mg steps at intervals of at least 4 weeks, up to a maximum of 15 mg once weekly. Wegovy is also increased gradually through the usual 0.25 mg, 0.5 mg, 1 mg, 1.7 mg and 2.4 mg weekly schedule.

This table is only an estimate and should not be used to switch treatment without clinical review. Some patients may need to restart at a lower dose, especially if they had side effects, have missed doses, or have had a treatment break.
For example, a patient who tolerated Mounjaro well and recently had their last injection may be considered for a higher Wegovy starting dose than someone who had significant nausea or vomiting. If side effects were difficult to manage, the clinician may recommend starting lower and increasing more gradually.
You should not use Mounjaro and Wegovy at the same time unless specifically instructed by a prescriber. Always follow the dose and timing advised by your clinician.
